Family Support Referrals

Health and social service providers can refer Indigenous families to our team for culturally grounded, strengths-based support. Our Family Wellness Navigators work alongside families to identify their priorities, access services, navigate systems, and build on existing strengths and supports.

Through a collaborative and family-centred approach, we help families develop personalized wellness plans that reflect their goals, cultural identity, and unique circumstances. Support may include service navigation, advocacy, coordination with service providers, assistance accessing community resources, and referrals to internal and external programs and services.

Our work is grounded in respect, cultural safety, self-determination, and relationship-building. We recognize that families are the experts in their own lives and strive to ensure their voices, choices, and aspirations guide every step of the process. We also support the continuity of cultural identity, family connections, and community belonging as essential foundations for long-term wellness.

This referral pathway is intended for health and social service professionals seeking culturally safe, community-based support for Indigenous children, youth, and families. We work collaboratively with referring providers while maintaining a family-led approach that respects each family's goals, choices, and strengths.
