CONTEXT
Integrated into the service continuum of Native Montreal, the Family Health Clinic offers its members culturally safe primary health care services as well as traditional healing services. Based on a model of collaboration and complementarity, the clinic’s services are developed in partnership with the public system, notably through the establishment of service corridors that facilitate access to care.
Indigenous populations experience significant health inequities, caused in part by the complexity of and lack of familiarity with the public service network, which often fails to adequately serve them. As such, the role of the Health Navigator is essential to ensuring culturally safe continuity of care within health and social services institutions and to having a positive impact on the overall health of the urban Indigenous population of Montreal.
POSITION OBJECTIVE
To ensure culturally safe accompaniment services tailored to the needs of members throughout their care journey. To improve access to culturally adapted and culturally safe health care and services for the urban Indigenous population of Greater Montreal.
The Health Navigator position reports to the Director of Health and Wellbeing and is primarily based at the Family Health Clinic. The position is also fully integrated into the broader Native Montreal team.
RESPONSIBILITIES
● Support and accompany members throughout their care pathways from a cultural safety perspective by coordinating appointments, administrative processes, and communication with involved partners.
○ Contribute to assessing members’ needs and to developing action plans, in collaboration with the medical team and Native Montreal’s prevention/intervention services.
○ Participate in the service intake process by ensuring reception, triage, and appropriate orientation of members within Native Montreal’s service continuum or toward external resources.
○ Accompany members in coordinating and planning their medical appointments and follow-ups.
○ Accompany members within the public health system when necessary, including the possibility of providing transportation support.
○ Assist members with health-related administrative procedures when required.
○ Advocate for Indigenous clients to ensure they receive appropriate, culturally adapted, and culturally safe care and services; support them in complaint processes when required.
○ Meet members in environments where they feel safe, including their homes or non-institutional settings when appropriate, to reduce barriers and foster trust.
● Support management in liaising with health system institutions and contribute to the development of service corridors with mutually beneficial objectives for Native Montreal members.
○ Continuously assess the level of cultural safety in care settings and recommend improvements to promote welcoming, respectful, and prejudice-free environments.
○ Raise awareness and educate care partners about Indigenous realities, practices, and cultural needs in order to strengthen cultural safety within services offered to members.
● Organize outreach and representation activities to promote the Family Health Clinic’s services within the community and among partners.
○ Collaborate with other departments or Indigenous organizations to organize community health activities (wellness days, prevention kiosks, health promotion activities, etc.).
● Write daily intervention notes, ensure rigorous record-keeping, and produce activity reports as required.
● Participate in data collection, continuous service improvement, and the adaptation or development of clinical and administrative tools.
● Participate in and collaborate in recommended training activities.
● Perform any other related duties as requested by their immediate supervisor.
REQUIRED PROFILE
Academic and Professional Experience
● Completion of a college diploma (DEC) in a field related to counseling, social services, personal development, community work, or support services, combined with three (3) years of relevant experience,
or
a university degree (BA) in a related field combined with one (1) year of relevant experience.
● Demonstrated experience in: health navigation, system navigation, intervention, accompaniment, or advocacy within complex service systems
Skills and Abilities
● Ability to adapt intervention strategies to the needs, realities, and cultural context of members.
● Strong capacity for communication, relationship-building grounded in respect, humility, and trust.
● Ability to maintain clear professional boundaries while providing supportive, culturally safe accompaniment.
● Creativity and professional autonomy in navigating complex systems and identifying solutions.
● Ability to work effectively as part of a multidisciplinary team, recognize others’ expertise, and contribute positively to collective work.
● Demonstrated trauma-informed, strengths-based practice, or a strong willingness to develop these competencies through training and supervision.
● Proficiency in French and English (oral and written).
Additional Requirements
● Indigenous identity and lived experience within Indigenous communities (asset). Non-Indigenous candidates should have experience in Indigenous contexts and a, demonstrated record of culturally safe practice.
● Comfort working in community-based and non-institutional settings
● Experience with relational databases and digital case-management tools.
● Valid driver’s licence.
● Must pass a criminal background check.
WORKING CONDITIONS
● Full-time position, 35 hours per week, with flexible scheduling based on members’ needs.
● Salary based on experience and organizational salary scale ($50,000–$76,081).
● Three weeks of vacation after the first year and two fixed weeks of vacation during the holiday period.
● 15 paid sick days per year.
● 5 paid days for family responsibilities.
