Wellness Navigator - Case Intervention

We are looking for a committed individual who is eager to ensure the well-being navigation of our members within Native Montreal.
ROLE & RESPONSIBILITIES
Under the supervision of the Director of Prevention and Intervention Services, the Wellness Navigator will primarily work with Indigenous youth and families, providing personalized support and helping them access the necessary services and opportunities to achieve their goals. This position involves both case management activities and active participation in community activities and group interventions in a culturally safe approach.
Specifically, the person will be expected to:
Case Management
Build trusting relationships with participants and their families to foster their engagement with the process, services, and support offered.
Collaborate with participants to develop a personalized action plan that reflects their autonomy, skills, needs, goals, and cultural values.
Continuously assess the strengths, risks, and needs of participants and adjust the action plan and services according to the progress made.
Provide guidance and direct participants to the necessary resources to promote their mental, emotional, physical, and spiritual health.
Collaborate with external stakeholders (i.e., health professionals, legal services, service providers, etc.) to offer comprehensive support.
Ensure accurate and compliant record-keeping.
Analyze assessment data to identify trends, deepen the understanding of needs, and adjust interventions accordingly.
Stay up to date with intervention practices and participate in professional training.
Community Outreach
Maintain a presence in the community to increase the visibility of services.
Inform potential participants about available services and resources;
Direct members to culturally relevant programs, intervention resources, and appropriate support systems.
Group Intervention Activities
Work closely with animation and coordination teams to ensure the smooth running of activities.
Build connections and trusting relationships with activity participants.
Identify signs of distress or problematic behaviors and intervene with care, calmness, openness, and solidarity.
Contribute to the evaluation and continuous improvement process of programs and activities that meet the emotional and developmental needs of participants.
The position holder is also expected to support Native Montreal's special activities and development projects.
EXPERIENCE AND REQUIRED SKILLS
Education and professional experience
Have completed a college diploma (DEC) in a program related to counseling, personal development or support services combined with three (3) years of relevant experience, or a university degree (BA) in a related field paired with one (1) year of relevant experience.
Experience working with Indigenous population, particularly youth and/or families living in urban settings.
Comfortable with technology and digital tools.
Proficiency in English with a good knowledge of French.
Knowledge of an Indigenous language (asset).

Additional expertise in intervention will be considered an asset: housing, child protection, victim support, addiction, food security.
*Must pass a criminal background check.
WHAT WE OFFER
Permanent position of 35 hours per week, with the possibility of evening and weekend work depending on program and participant needs. The schedule is flexible and adjusted on a case-by-case basis.
Salary of $50,000 to $76,081, depending on experience and internal equity.
Three (3) weeks of vacation after the first year and two (2) fixed vacation weeks during the Christmas holidays.
Fifteen (15) paid wellness days per year.
Five (5) paid family obligation days.
